To get started, simply download, print and complete the appropriate direct deposit form listed below.

• Payroll Direct Deposit Form – Use this form to sign up for direct deposit of your paycheck and deliver completed form to your payroll department.
• Social Security and other Government Payments Direct Deposit – If you are a government employee, use this form to sign up for direct deposit of your paycheck. If you receive Social Security, Supplemental Security Income, VA Compensation or Pension, or other like payments, use our Government Direct Deposit Form to sign up for direct deposit of these payments. After form is completed, drop by any HeritageBank Office nearest you and we'll forward it to the appropriate agency responsible for your payments.
